What is Eyelid Surgery?

Eyelid Surgery, medically termed Blepharoplasty, is a highly customized, cosmetic operation designed to address eyelid problems that may impact appearance and/or vision and even discomfort. It may be performed solely on the lower eyelid (Lower Blepharoplasty) or upper eyelid (Upper Blepharoplasty) and sometimes on both eyelids.

Indications of Eyelid Surgery

  • Ptosis- It is the hanging down of the upper eyelid skin of one or both eyes. It can impair peripheral or central vision. If one’s upper eyelid skin is heavy and droops, the face looks uneven and if both, the eyes look tired, sad, or even angry, even if one is not feeling such. With upper eyelid surgery, this condition can be addressed.
  • Loose, excess eyelid skin- Eyelid skin is the thinnest and most delicate skin and with advancing age, the reduction in skin’s elasticity along with gravity pulling the skin leads to loose, extra eyelid skin. If the upper eyelid skin is in excess, it can hang down and obstruct vision and if lower eyelid skin is in excess, it results in under-eye bags or puffiness. An eyelid surgery can help remove extra skin from the eyelids and even help get rid of surplus fatty deposits near the eyelid while tightening eye muscles and tissues.
  • Ectropion and entropion– Ectropion is the downward pulling and outward turning of the lower eyelid that occurs because of aging, tumor, trauma, skin burn, or skin disease. It makes eyes dry, watery, and sensitive to wind and light. Entropion is the inward turning of the lower eyelid skin towards the eyeball due to infection, age, or scarring inside the eyelid. It makes the eyes watery and irritated and may even increase the risk of corneal infection. What to Expect from an Eyelid Surgery?

Eyelid surgery is an outpatient procedure, lasting for about 45 minutes to 2 hours, and executed under the effect of local anesthesia. Before the surgery, the surgeon conducts a thorough eye exam, performs pre-surgical medical evaluation/lab tests, and talks about what the procedure entails and its benefits and risks. Photographs may be clicked from different angles of both the eyes and the patient’s vision is even tested. The patient is instructed to avoid intake of certain medications and smoking as well as arrange for someone to drive home and take care of the first night following surgery.

During an eyelid surgery, a hidden incision is made in the eyelid. For upper blepharoplasty, the incision is made in the eyelid’s natural crease whereas for lower blepharoplasty, the incision is made below the eyelash line or inside the lower eyelid. Through the incisions, the surgeon removes excessive eyelid skin and redistributes or removes the protruding fat. In some cases, the eyelid skin is repositioned or lifted. Finally, the incisions are closed using sutures that stay for about one week.

Following an eyelid surgery, it’s normal to see slight, temporary bruising and swelling as well as unexpected tightness in the eyelid skin. To alleviate swelling and discomfort, a cold compress can be applied. For quick, smooth, and optimal healing of newly rejuvenated eyelids, the patient is instructed to take proper rest with the head elevated and avoid facial exercise or high-impact physical activities for up to 4 weeks.
